At Home Network, Inc. is a full service home care agency that offers varied and specialized care provided by a team of highly trained, conscientious, and caring professionals.

 

All of our patients are referred to At Home Network, Inc. by their physician.  We work closely with the physician to plan our patient’s home care services from the initial referral to the discharge.

 

We are proud to offer the following services provided by our highly qualified, professionally trained staff:

 

Skilled Nursing– performed by our experienced nurses, home and patient evaluation, wound care, IV infusion, and, education.

 

Rehabilitation Services– Comprehensive rehabilitation programs that are developed through consultation with the physician and carried out by our registered physical therapists, occupational therapists, and speech language pathologists.

 

Medical Social Services– Professional social workers are an integral part of our team, providing our patients with a well rounded continuum of care.  These professional counselors assist our patients and their families to cope with the stresses of their illness and all that it entails.

 

Nutritional Services– Our registered dieticians will provide a personalized assessment of the patient’s dietary needs and recommend a diet that is aimed towards their present nutritional requirements.

 

Personal Care– performed by our home health aide specialist.

 

We have been in the Oakland county area for 13 years and the Genesee county area for the last 6 years. We have specialized programs for CHF, COPD, diabetes, pain management, joint replacement, and most recently, we have initiated a new Psychiatric program in the Genesee county area.  We are the preferred provider with Beaumont Infusion to take care of their challenging patients in the Metro Detroit area.  We are partnering with Diplomat Pharmacy in Flint to take care of their patients for infusion therapy.

 

We are a CHAP Accredited and Medicare Certified Agency.    Besides Medicare, we provide services for patients with BCBS, Health Plus, PPO, PPOM, and auto insurance.  We are also a member of the Allscripts program referrals system.  Our clinical documentation is done using laptops with Allscripts paperless charting system.
